Outreach Services
‘Chat to PAT’ is a purpose-built mobile clinic that provides wrap around support to marginalised Canberrans who can’t easily access health services, including people who are homeless or experiencing significant disadvantage.
The service is provided by a multi-disciplinary team which includes GPs, nurses, counsellors and mental health practitioners. The team provides primary health, mental health, drug and alcohol support and case management.
Overall, PAT provides a safe and trustworthy place for people to access care across the ACT, five days a week at six different locations, including large public housing complexes, Veterans Park and the Early Morning Centre.
Directions Health’s GPs, nurses, counsellors and mental health practitioners provide additional outreach services around the ACT each week, in addition to ‘Chat to PAT’.
This includes fortnightly counselling services at headspace Tuggeranong and headspace Canberra City, weekly GP and nursing services at CAHMA in Belconnen, a weekly nursing service at the NSP in Canberra City and Teen Clinic which operates in Queanbeyan and Conder weekly during school terms.
